Who We Are

Primary Care Providers for a Healthy Feliciana, Inc. (PCPFHF) is a 501(c)3 not-for-profit organization that has served the residents of East Feliciana Parish since 1999, and West Baton Rouge Parish since 2008, providing quality, affordable health care regardless of ability to pay. PCPFHF is comprised of five stand alone clinics in East Feliciana Parish, four of which are adjacent to each of the public schools within the East Feliciana Parish Public School System.

Livingston parish residents can now access primary care, dental care and behavioral health care in two locations – RKM Primary Care Livingston at 29437 Hwy. 63 (S. Frost Road), Livingston and our newest location RKM Springfield, 27124 Highway 42, Springfield right next to Hometown Pharmacy! RKM also provides behavioral health services in 34 Livingston Parish Schools.

Eleven schools in the Tangipahoa Parish School System have access to behavioral health services. All school services require written parental consent.

All PCPFHF Health Centers are Federally Qualified Health Centers and Primary Care Medical Homes. Each location is Joint Commission Accredited and covered by the Federal Torte Claim Act Medical Malpractice Coverage for Health Centers for 2015. This model of care ensures a comprehensive approach to meeting our patients’ needs:

  • Comprehensive CarePCPFHF uses a diverse team of providers to provide the full complement of comprehensive primary health care services for all ages. We also provide comprehensive and integrated oral health and behavioral health/substance abuse services, access to a discount drug program, transportation and a wide range of enabling services.
  • Patient-Centered CarePCPFHF providers’ provide relationship-based care to our patients. We not only respect our patient’s unique needs and preferences, but we also encourage them to actively participate in their care.
  • Coordinated CarePCPFHF helps all patients access the care they need, whether we provide it on-site or we have to refer them to specialty services.
  • Accessible ServicesPCPFHF ensures that our patients have access to care through flexible appointments Monday-Saturday and 24/7 after hour on-call coverage.
  • Quality and SafetyPCPFHF provides a systems-based approach to safe, quality care. We are continually engaging in quality improvement initiatives, which include input from our patients.



NOVEMBER 9, 2015
WBR Primary Care will open in its new location 203 Allendale Drive, Port Allen, La.
The new location is at the corner of Court Street and Allendale Drive on the west side of the railroad tracks in Port Allen. We are moving into to a temporary building at this address. This building will be an improvement in many ways. First it has better parking and a bit more elbow room for patient care! We will have space for dental services which we have not been able to provide before and expanded behavioral health services. We will occupy this temporary building while we await the completion of our 8,629 Square Foot building sometime in March 2016. RKM Primary Care is partnering with the East Feliciana Parish Assessors Office to sponsor our third Holiday Market. Vendor spaces are $10 and will be out under the big tent on the parking lot this year for great visibility. We will be selling gumbo. All proceeds will go to the Assess and Serve Program! To operate and maintain nonprofit primary care, patient centered medical homes that provide coordinated care encompassing the whole person – physical, mental and spiritual – with a focus on health education, illness prevention, health maintenance and wellness.


For all residents of the PCPFHF service area to have access to affordable, quality health and wellness services, enabling them to achieve quality of life for themselves and their families.
